Yá'át'ééh! Please see the details listed in the flyer regarding the Diabetes Care and Prevention Team and Hozho’go Iina 365 Program virtual event! You have 4 days to complete the event. However, either the 5K (3.1 miles) walk/run must be completed on only 1 day of the dates listed.
The link to join the event is below. You must join the event directly using the link in order to receive ANY incentives. You must provide proof of completion by posting a picture of your Strava route or fitness watch data with miles clearly stated from the walk/run into the community discussion board on the club page. Participants will have the chance to be drawn in a raffle for either a meal basket or fitness watch. The winners will be contacted directly for coordination for pick-up or delivery. Only Navajo Nation participants residing in the southeastern part of Utah (Montezuma Creek, Blanding, Monument Valley, Navajo Mountain) will be entered into the drawing which will occur the week of November 29th through December 3rd. Each participant will be given a free t-shirt for participating and completing the event. The link is below to complete the form for incentives. You must complete the form in order to receive a t-shirt. YOU ARE RESPONSIBLE FOR PICK-UP OF YOUR T-SHIRT. We are NOT mailing any t-shirts this year. The form will have information on site pick-up for each clinic of our service area only (Montezuma Creek, Blanding, Monument Valley, Navajo Mountain).
